Exit 9: Care Management:
Before going on a road trip, it is essential that your car is working properly. This can help you avoid car troubles or breakdowns along your journey, and help you arrive safely at your destination. Do you get your car serviced regularly or do you only take it to your mechanic when it is not running properly? There are recommended preventive maintenance schedules, computer diagnostic tests for symptoms, and replacement parts for cars. Some mechanics are generalists and others specialize in certain types of cars. Most are certified so you can rest assured that they know how to service your car properly.
Similarly, a SBHC offers an appropriate scope of services and staffing for the student population that it serves. Students that use SBHCs receive screening, preventive services, diagnostic testing, and clinical interventions to keep them on the road to health and optimal learning. The clinical staff who manage the students’ care need to follow clinical guidelines and evidence based practices, properly record, document, and code the services provided, communicate with students and families, and measure outcomes. Exit 9 contains a wide range of care management related training presentations, resource materials, policies and procedures, samples, and links to help you successfully navigate the delivery of primary care, dental and mental health care, and nutrition services in your SBHC.
