Patient Centered Transformation in School Based Health Care: Lessons Learned

ABSTRACT SUMMARY

The patient centered transformation process is both challenging and rewarding. Through our PC-SBHC pilot project we have identified many of these challenges and developed tools for SBHCs to deliver patient centered quality care. Participants will practice utilizing the tools to create their own transformation.

ABSTRACT DESCRIPTION

The transformation process provided by the Patient Centered Medical Home is well documented, but some of its elements are square pegs for even the most well-rounded of SBHCs. Five sites in New York City piloted Patient Centered School-Based Health Care standards, alternative quality standards that maintain the rigor of PCMH, but add elements that reflect the preventive care and population management strengths of SBHCs. Many of the lessons learned in the pilot process will be able to assist SBHCs nationwide to improve the quality of patient centered care in their home sites. We will briefly describe the pilot standards and project to give a context for the lessons learned. We will include the areas of congruity and divergence between the PC-SBHC and the NCQA 2014 standards. We will identify, with audience participation, the primary challenges, such as care coordination, and documentation of primary care provider status, that SBHCs face in the traditional PCMH process and how the PC-SBHC standards address them. The workshop is designed to assist the audience in identifying and defining the important elements of patient centered transformation that they can incorporate into their practice with or without a state or national recognition process. We will share tools that they can use to assess their own practice and assist them to design an individualized transformation work plan based on PC-SBHC standards. Finally, we will explore methods of evaluating the impact of transformation on quality of care. Audience comments, questions and discussion will be encouraged.
