Achieving Community Health Outcomes through School Health Systems
ABSTRACT SUMMARY
Schools are an ideal place to conduct population health services. Health Centers in Schools (HCS) serves East Baton Rouge Parish (LA) schools. Over 28 years, HCS has built a systematic and comprehensive approach to population-level care for its students. Panelists will share how the community’s considerable health assets are leveraged to deliver system-wide hearing and vision screening, influenza immunizations, asthma education and care management, obesity education, and nutrition services for at-risk children and youth.

Schools are an appropriate—and ideal—place to conduct population health services. Health Centers in Schools (HCS) is a not-for-profit healthcare organization that serves the entire school system of East Baton Rouge Parish in Louisiana. Panelists will describe how, over the organization’s 28-year history, it has built a systematic and comprehensive approach to population-level care for its students. HCS leadership will share how the community’s considerable health assets are leveraged to deliver systemwide hearing and vision screening, influenza immunizations, asthma education and care management, obesity education, and nutrition services for at-risk children and youth. Program highlights will include: 1) mobile programs that offer optometry and nutrition education, 2) the adoption of tele-health technology to link school nurses with the programs 7 school-based primary care centers, and 3) health education strategies designed and delivered by a student alliance council through social media.
