MHPET TEAM LEADER INSTRUCTIONS

TO COMPLETE THE MHPET

  1. Review the MHPET Assessment Tool
  1. Select a team. You must have a total of at least three but not more than eight team members. If you have fewer than three, your final score will not be computed.
  1. Each team member must have his/her own NASBHC ID to log onto the survey. If the team member does not have this ID, s/he should call 202-638-5872 or 888-286-8727 to get an ID number.  In order to protect confidentiality, NASBHC will only release ID numbers to that individual.
  1. Access the MHPET by clicking on the MHPET login button above.  The team leader completes Set One (team leader must complete Set One first).

  2. The team leader notifies other team members to begin Set One.  Click here for a letter providing team members with instructions for completing the MHPET(this will open a new window).  This letter can be printed or copied into an email.   All team members should complete Set One within two weeks.
  1. When all team members have completed Set One, the MHPET team leader closes out Set One (directions will be given).
  1. Convene the team to review the automatically calculated scores. As a team, develop an action plan based on this information and implement an action plan.
  1. Complete Set Two: 
    a.   Three to six months later, arrange for the same team to complete Set Two. (Return to step 5 above.)
    b.   If you are unable to have the same team of raters, attempt to have the same number and roles represented on the team.
  1. As a team, compare the results from both sets to look for improvement or determine a need for further action.
