You must have your own ALLIANCE ID to log onto the survey. If you do not have this ID, you should call 202-638-5872 or 888-286-8727 to get an ID number. In order to protect confidentiality, The School-Based Health Alliance will only release ID numbers to that individual.
NOTE: If your program is completing Set Two of the MHPET and you are a new team member (did not complete Set One), you must contact Alicia Newell at (202-638-5872 or 888-286-8727) to be added to your program’s team.
Access the MHPET by clicking on the MHPET login button above.
Complete Set One.
When all team members have completed Set One, your MHPET team leader will close Set One and begin facilitating the action planning and implementation process.
Three to six months later, you may be asked to Complete Set Two of the MHPET. This will allow your team to compare the results from both sets to look for improvement or determine a need for further action.
